Abstract:

Osteoporosis is a skeletal disease characterized by low bone mass and micro architectural deterioration of bone tissue with a consequent increase in bone fragility and susceptibility to fracture (WHO 1994). Osteoporosis is an important health issue around the world that estimates to affect more than 200 million people energy year worldwide.

In India around 50 million population of Indian are either osteoporotic or have low bone mineral density (BMD).

Among Indian woman age 30-60 years, BMD at all skeletal sites were much lower than values from other countries reported, with a high incidence of osteopenia (52%) and osteoporosis (29%), may be due to inadequate nutrition.

Material & Method

This is a prospective study conducted at PGIMER, both in OPD and ATC ward, among all patients admitted with the history of fracture involving neck of femur and intertrochantric, excluding metastatic hip fractures conservatively treated fractures,  patients in which biopsy could not be taken and those with secondary osteoporosis.

Calcium profile, serum cretinine, urea, bilirubin, AST and ALT levels were measured. Hb ALC, IPTH, Vit. D were analysed.

During peri-operative procedure, bone biopsy specimen measuring 0.5-1 cm size was sampled and sent from histological assessments.

Results

Post fragility fracture in seven patients (15.6%) was previous history of fragility fracture (due to minor trauma) and rest 38 patients (84.4%) had no history of such type of fracture. Corrected calcium level 20 patients (44.4%) were showed abnormal calcium level below 8.8 mg/dl and rest 25 patients (55.6%) were showed normal value.

Ca xp product value 19 patients (42/2%) showed values below 27 mg/dl and rest 26 patients (57.8%) showed normal values.

Serum Vit. D level normal level is 11.1 – 42.9 mg/ml  22 patients (48.9%) showed values below  11.1 mg/ml. and rest 23 patients (51.1%) showed normal serum vit D level.

Conclusion – Vit. D deficiency is highly prevelant among fragility hip fractures patients in north Indian population.

Abstract:

The OActive project is a European project that studies knee OA. It intends to make a significant leap forward adopting a multi-scale holistic analysis where patient-specific information from various levels, including molecular (e.g. biochemical/inflammatory biomarkers), cell, tissue and whole body, will be integrated and combined with information from other sources such as, environmental, behavioural and social risk factors to generate robust predictors for new personalised interventions for delaying onset and/or slowing down progression of OA.
OActive targets patient-specific OA prediction and interventions by using a combination of mechanistic computational models, simulations and big data analytics. Once constructed, these models will be used to simulate and predict optimal treatments, better diagnostics, and improved patient outcomes, overcoming the limitation of the current treatment interventions, Augmented Reality (AR) empowered interventions will be developed in a personalised framework allowing patients to experience the treatment as more enjoyable, resulting in greater motivation, engagement, and training adherence. The AR element will also be helpful for the therapists for validating the patients’ progress and allow them a more adaptive rehabilitation therapy in terms of flexible interactive content.
Objetives:
The OActive scientific and technological objectives focus on the development of computer-based predictive models and simulation for understanding the onset and progression of a major disease like OA.

Introduction

PRP is an autologous blood product that is created by first obtaining a small amount of blood through peripheral venesection, concentrating that blood sample through centrifugation, and then administering the concentrated plasma product back into the patient via an intra-articular (IA) injection. Autologous platelet-rich plasma has emerged as a treatment option for treatments for osteoarthritis.

Objective

The aim of this study is to evaluate the efficacy of single PRP injection to reduce the severity of OA in Knee joint

Methods

Thirty three patients with primary and secondary knee osteoarthritis who met the study criteria received single platelet-rich plasma injections in the affected knee at medical unit, Teaching Hospital, Batticaloa during the year 2018. Severity of the OA was assessed using Western Ontario and McMaster Universities Osteoarthritis Index (WOMc) just before, one month, three months and six months after the PRP injection. Paired sample t-test  was used for data analysis.

Results

There were 33 patients with Osteoartirtis. Mean age of the patients was 61.3 (S.D. 12).  Osteoarthritis grade 1 to 4 was 9.1%, 48.5%, 30.3% and 12.1% consecutively. Mean Western Ontario and McMaster Universities Osteoarthritis Index (WOMc) score at presentation was 65.2 (59-71).  One month, three months and six months WOMc scores were 25 (17-33), 24 (14-34), 24 (13-35) respectively. Paired sample t-test showed a significant improvement in WOMc score in 1 month (p<0.001), 3 months (p< 0.001), and 6 months (p<0.001). But there was no significant improvement from 1 month to 6 month post PRP injections. No PRP injection specific complications were reported.

Discussion

Single injection PRP is a good and reliable option to relive the symptom and sign burden in order to reduce the severity and suffering from KJ OA. Age and initial OA severity is not affect the outcome of PRP injection. Six monthly PRP injections can be a justifiable option as WOMc index starts to show a rise in severity even-though it is not statistically significant.
